Immunization Requirements

The Board of Regents sets the required immunizations for all Georgia Colleges and Universities. The immunizations required by Georgia Tech are different from other universities, however, these requirements must be met before you can register for classes. When you are accepted at Georgia Tech or request cross-registration at a Georgia Tech campus you are automatically placed on hold and will not be allowed to register for classes until you meet the immunization requirements.  Please click on the link to access the necessary immunization forms.

The form is an editable PDF. Open the file in Adobe Acrobat and type in your information in the Student Information Section. It is especially important to type in your GTID. You should type in any other requested information where possible, including a cell phone number.

Please read all of the instructions. Complete all forms, include necessary signatures & dates and fax to 912-966-7836 as soon as possible.

Steps:

• Complete, sign, and date the Medical Entrance Form (page 1). Parents must sign if the student is under 18 years of age.
• Complete the Certificate of Immunization (Required). The physician/nurse must sign, date, and stamp the form. The dates must be legible or you will have to resubmit all your forms which will delay removing your hold.
• Complete the TB Screening Form (required) within 3 months of starting classes. A healthcare provider must sign and date the form. If you are at risk, you must get a TB Skin Test in the United States. TB Screening, TB Skin, and/or X-ray MUST be completed in the United States if you are from any country other than the U.S. or Canada.
• Fax all forms to 912-966-7836.
• Verify if your registration hold has been removed byvisiting: http://mystuff.gatech.edu/health

Checklist:

• Medical Entrance Form – Completed, signed & dated
• Certificate of Immunization Required -Completed- readable-signed & dated
• TB Screening Form-Completed & signed & dated (TB Screening, TB Skin, and/or X-ray MUST be completed in the United States if you are from any country other than the U.S. or Canada)
• If at Risk-TB Skin Test-Completed & signed & dated
